 How to Apply for the 12-month Resident Program

1. Read the Tabitha Resident Expectations.

2. Complete an application by printing out the below form or by fill out the application through Google Forms: https://forms.gle/koxynf24ZEckRUu18. Applicants who apply using Google Forms need to read the Legal Release included at the end of the application below.

3. Return the completed printed application to Tabitha Ministry in one of the following ways:

• Email completed application to tabithaministryncinfo@gmail.com.
• Fax completed application to (336) 370-6321.
• Mail completed application to Tabitha Ministry, P.O. Box 514, Summerfield, NC 27358.
